The National Database and Registration Authority (NADRA) has specified the necessary documentation for parents requesting a Juvenile Card for their children for the first time. Applicants are required to submit the identity card number of a legal guardian or a parent during the process.
If a child already possesses a B-Form, the parent must provide that specific document to the authority. A verified Computerized Birth Certificate containing a CRMS number is also mandatory for the application.
For children born outside of Pakistan, the agency requires a detailed foreign birth certificate, an S1 form, or a certificate of naturalization or citizenship. Biometric verification of the child's legal guardian or a parent is a prerequisite for the issuance of the card.
The child must be physically present at the time of the application. Parents and legal guardians may submit these applications at any nearby NADRA Registration Center.
The Juvenile Card functions as the official identity document for all children under the age of 18.
